Nominees are accepted and reviewed for the following criteria:
- Display exemplary leadership and diligence in promoting public health.
- Have made remarkable contributions and support in fostering public health programs.
- Work or actions will have impacted the community’s health in a positive way.
- Must live and/or work in the Lake Cumberland District.
This award is named in honor of its first recipient, the late Dr. Hossein Fallahzadeh. He was a long-time surgeon in Somerset, a board member for the District Board of Health as well as a long-serving member of the Pulaski County Local Board of Health, and a staunch supporter of public health in the Lake Cumberland District.
