1. Family Support Worker I/Home Visitor

The Lake Cumberland Health Department is accepting applications a full-time Family Support Worker I/Home Visitor based in Pulaski County.

General Duties include:  This position serves under general direction of the Program Coordinator and/or appropriate supervisor.  Responsibilities for this position include, but are not limited to: Conduct regular planned visits to establish a trusting rapport for eligible HANDS participant/s to assist parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits.  Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers.  Initiate and maintain regular and long-term contact and provide support to parents during the prenatal period and extending up to, but not inclusive of the child’s second birthday, following parent visit assessment.  The responsibilities include home visitations that may take place in the client’s home or another community site if justified, that include (a) monitoring of the child’s, mother’s, and family’s progress by making referrals to community resources; such as prenatal health services, school, head start, etc.  Tracking appointments to ensure they are being kept, performing follow-up services as identified and performing periodic evaluations of the participants’ changing needs, (b) preparation and maintenance of case records which shall be documented with contacts, services needed, reports and progress, (c) consultations and (d) crisis assistance.  Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.

Minimum Education, Training or Experience:  High School Diploma or GED.

Must have telephone accessibility

Must have reliable transportation and valid driver’s license

Criminal Background Check

On the job training requirement needed within 6 months of hire in order to advance:

HANDS home visitor core training (Must be completed before family contact can occur)

HANDS wrap-around required training (Specific topical areas checklist)

CPR Certification

Working knowledge of geographic area and/or ability to locate participants and community resources with use of maps, contacts and directions provided.

Starting Salary:  $13.50. 5% raise after 6-month probationary period. Grade 10

2. HANDS Specialist 1 

The Lake Cumberland District Health Department is accepting applications for a full-time HANDS Specialist I in Pulaski County.

General Duties include:  This position serves under the direction of the Director or other appropriate coordinator/supervisor assigned.  Responsibilities for this position include, but are not limited to; Conduct parent visit assessment/survey within 30 days of receiving, after review of prenatal or birth records from the hospital or clinic, to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three (3) years of age.  Conduct interviews to promote outreach activities in family homes or community setting to educate parents in coping techniques and to take an active role in accessing resources or assistance with referrals for appropriate education and nutrition of their child.  May develop and distribute informational materials that meet program guidelines. Keeps accurate logs of service visits, schedules and charting, prepare reports and analyses set forth in Health Access Nurturing Development Services (HANDS) program policy and guidelines. Interpret laws, regulations, policies and procedures and provide administrative assistance or guidance to departmental staff, which may include but is not limited to; monitor, evaluate and participate in modifications regarding program requirements.  Review records and reports submitted by program requests or agency staff and ensure compliance with federal and state law regulation and policies.  May conduct ongoing home visitation on an as needed basis.

Minimum Education, Training or Experience:  Bachelor’s Degree in Social Work, Nursing, Early Childhood Education

OR

Bachelor’s Degree in Social or Behavioral Science or related field with one (1) year experience performing case management services

Substitution for Education:

Associate Degree in Nursing with licensure in Kentucky or compact state and two (2) years of public health experience.

Special Program Requirements:  Meets education requirement per 902 KAR 4:120.

Starting Salary:  $17.04/hour. 5% increment given after six-month probationary period. Grade 14

3. Health Educator I 

Lake Cumberland District Health Department is accepting applications for a full-time Health Educator I.

General Duties include:  This position serves under the direction of the Health Education Director.  Responsibilities for this position include, but are not limited to: Designing, adapting and implementing targeted health education programs and policies for our community.  In this role, your duties would be to collect and analyze data and develop health education initiatives aimed at promoting healthy lifestyles, wellness and a healthy environment.   Meet with members of the community to establish a working relationship and become familiar with available community resources.  Assist in exchanging information with various organizations and agencies concerning health needs of the community and assist in assembling materials to be utilized.  Develop plans and policies toward achieving health education objectives, and monitoring the progress.  Facilitate health education events, workshops and presentations.  Participate as a team member, having a basic understanding of the team process.  Demonstrate an awareness of public service announcements (PSA) and assist in writing public service announcements for radio and television.

5. RN-Public Health Nurse I for HANDS program

The Lake Cumberland District Health Department is accepting applications for full-time RN– Public Health Nurse I positions within the HANDS program to provide services in Pulaski County.

General Duties include:  Responsibilities for this position include, but are not limited to; conducting referral screenings and parent surveys on eligible families interested in participating in the HANDS program. Provide professional RN home visits and documentation of visits for participating families. Attend meetings and trainings as required and must be able to collaborate with appropriate local agencies.

LCDHD is also accepting applications to establish a register for local health nurse positions within our 10-county service area.
