  1. Family Support Worker I/ Home Visitor

The Lake Cumberland Health Department is accepting applications a full-time Family Support Worker I/Home Visitor based in Casey County- may cover other counties within our service area as needed.

General Duties include:  Responsibilities for this position include, but are not limited to; Conduct regular planned visits to establish a trusting rapport for eligible HANDS participant/s to assist parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits.  Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers.  Initiate and maintain regular and long-term contact and provide support to parents during the prenatal period and extending up to, but not inclusive of the child’s second birthday, following parent visit assessment.  The responsibilities include home visitations that may take place in the client’s home or another community site if justified, that include (a) monitoring of the child’s, mother’s and family’s progress by making referrals to community resources; such as prenatal health services, school, head start, etc.  Tracking appointments to ensure they are being kept, performing follow-up services as identified and performing periodic evaluations of the participants’ changing needs, (b) preparation and maintenance of case records which shall be documented with contacts, services needed, reports and progress, (c) consultations and (d) crisis assistance.  Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.

Minimum Education, Training or Experience:  High School Diploma or GED and experience in a Social or other health related field such as but not limited to, working with children, teens and adults in a daycare, medical office or facility, school etc., that provides a medical or a community support service.

Substitution for Education, Training or Experience:  Education from an accredited college or university in a Health related or Social and Behavioral Science field may substitute for the required experience.

Special Requirements:  Applicants must be 18 years of age or over, have telephone accessibility, reliable transportation and a valid Driver’s License.

On the job training requirement needed within 6 months of hire in order to advance:

  • HANDS home visitor core training (Must be completed before family contact can occur)
  • HANDS wrap-around required training (Specific topical areas checklist)
  • CPR Certification
  • Working knowledge of geographic area and/or ability to locate participants and community resources with use of maps, contacts and directions provided.

Starting Salary:  $12.29. Plus five (5) percent increase after successful completion of six-month probationary period.

2. Local Health Nurse 1 (HANDS)

The Lake Cumberland District Health Department is accepting applications for full-time RNLocal Health Nurse I positions within the HANDS program to provide services in Casey County– may cover other counties in our service area as needed.

General Duties include:  Responsibilities for this position include, but are not limited to; conducting referral screenings and parent surveys on eligible families interested in participating in the HANDS program. Provide professional RN home visits and documentation of visits for participating families. Attend meetings and trainings as required and must be able to collaborate with appropriate local agencies.

Minimum Education, Training or Experience:  See special requirements.

 Special Requirements: (Age, Licensure, Regulation, Etc.)  Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041.

Starting Salary: $19.80. 5% increase after successful completion of six-month probationary period.

3. Public Health HANDS Specialist 1

The Lake Cumberland District Health Department is accepting applications to fill two (2) full-time positions for Public Health HANDS Specialist- “Parent Visitor” covering Casey or Taylor County.

General Duties include:  This position serves under the direction of the Director or other appropriate coordinator/supervisor assigned.  Responsibilities for this position include, but are not limited to; Conduct parent visit assessment/survey within 30 days of receiving, after review of prenatal or birth records from the hospital or clinic, to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three (3) years of age.  Conduct interviews to promote outreach activities in family homes or community setting to educate parents in coping techniques and to take an active role in accessing resources or assistance with referrals for appropriate education and nutrition of their child.  May develop and distribute informational materials that meet program guidelines. Keeps accurate logs of service visits, schedules and charting, prepare reports and analyses set forth in Health Access Nurturing Development Services (HANDS) program policy and guidelines. Interpret laws, regulations, policies and procedures and provide administrative assistance or guidance to departmental staff, which may include but is not limited to; monitor, evaluate and participate in modifications regarding program requirements.  Review records and reports submitted by program requests or agency staff and ensure compliance with federal and state law regulation and policies.  May conduct ongoing home visitation on an as needed basis.

Minimum Education, Training or Experience:  Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or Elementary Education.

OR

Bachelor’s Degree in Social or Behavioral Science or related field with one (1) year experience performing case management services or a Master’s Degree in Human Services may substitute for the one (1) year of experience.

Substitution for Education:  Associate Degree in Early Childhood Development and two (2) years of Public Health, early childhood or education experience.

OR

Associate Degree in Nursing with Licensure in Kentucky or compact state and two (2) years of public health experience.

Special Program Requirements:  Meets education requirement per 902 KAR 4:120.

Starting Salary:  $11.40.  Five (5) percent increase after successful completion of six-month probationary period.
