The Lake Cumberland District Health Department is accepting applications for a full-time Public Health Nurse I based in Russell County within the Lake Cumberland District.
General Duties Include: Provides and coordinates community-wide diabetes prevention and management outreach through public education, media efforts, health fairs, presentations, and distribution of prevention materials aligned with the Kentucky State Diabetes Program. Delivers evidence-based group programs that promote behavior change for individuals and families affected by diabetes or prediabetes, including DSMES services such as weight loss, nutrition classes, support groups, and educational events. Serves as a lead facilitator for Diabetes Prevention Program (DPP) activities, ensuring compliance with NDPP standards, assisting with biannual reporting, participant care planning, chart audits, and quality improvement efforts. Develops and delivers diabetes education presentations, provides continuing education for healthcare providers, maintains CPR/First Aid certification, and mentors professionals pursuing Licensed Diabetes Educator credentials. Supports coalition-building, community campaigns, and partnerships through involvement with the Kentucky Diabetes Network, while contributing to data collection, grant activities, staff training, and ongoing program improvement initiatives.
Minimum Education, Training or Experience: Must have RN license in Kentucky or compact state
or provisional license in Kentucky, meeting all requirements of KRS 314.041 and approval to practice as Registered Nurse Applicant (RNA)
Starting Salary: $20.30
Merit Salary/Position Grade: 17

The Lake Cumberland District Health Department is accepting applications for a full-time Contracted Public Health Program Manager.
General Duties Include: The Program Manager will provide strategic leadership for a new regional initiative focused on improving health outcomes across the Lake Cumberland region by building an innovative program from the ground up through the development of implementation strategies, establishment of operational systems, and leadership of regional efforts to reduce obesity and diabetes using collaborative, evidence-based approaches. The position is responsible for leading strategic planning efforts; developing and implementing program goals, objectives, and work plans; convening a diverse cross-sector partnership representing all counties within the Area Development District (ADD); and establishing and cultivating relationships with healthcare providers, hospitals, community organizations, educational institutions, employers, local governments, faith-based organizations, and other stakeholders to create a strong regional network of partners. During the initial phase of the initiative, the Program Manager will lead the development of the Regional Community Hub (RCH) by recruiting, onboarding, and actively engaging a minimum of twenty (20) cross-sector partner organizations committed to advancing chronic disease prevention and improving community health while fostering collaboration among partners, facilitating regular coalition and stakeholder meetings, and ensuring meaningful engagement across the ten-county region. Serving as the primary public representative for the initiative, the Program Manager will deliver presentations to community groups, elected officials, healthcare leaders, and partner organizations to increase awareness, promote community engagement, and strengthen regional collaboration. Additional responsibilities include overseeing grant implementation, ensuring compliance with funding requirements, monitoring program performance and outcomes, preparing reports for leadership and funding agencies, identifying opportunities for long-term sustainability and future funding, and continuously evaluating program strategies to maximize regional impact.
