Helping HandsThe Diabetes Centers of Excellence is a free program that allows participants to work one-on-one with a Diabetes Educator/Care Coordinator who helps coordinate the individual’s diabetes care with their health care provider and also teaches diabetes self-management skills.


The Lake Cumberland District Health Department is proud to have been chosen as one of six sites for the Diabetes Centers of Excellence grant in 2006.  The goal of this program is to improve diabetes control and prevent complications.  Currently the Diabetes Centers of Excellence are operating in Clinton, Cumberland, McCreary, and Wayne counties.  This service may be extended to the remaining six counties in the near future.  If you have questions you may call 606-678-4761 EXT 157.  Our staff is ready to lend a helping hand!
